Once you've completed this step, your Family Advocate and Health Advocate will review it for completion and contact you to schedule an Acceptance Visit.

If you are missing any information, or anything further is required from you, your Advocate(s) will notify you during that call. If you have any additional questions about this process, or the program, your Advocate(s) can answer those during your meeting as well.
